Recent health care transformation, and federal payment changes have created opportunities for improved nutrition care in Oregon. In this new environment, and using available technology, nutrition risk screening in ambulatory and acute care settings can help identify at-risk patients. Early identification of those who experience nutritional decline are intended to help reduce physician office time, reduce acute care admissions, better prepare and recover from surgery, reduce or prevent clinical malnutrition and identify and code the nutritional and medical diagnosis of protein-calorie malnutrition. Efforts are also underway to push-out electronically-based resources to the public so they can better engage and address their nutritional concerns with their health care providers.
This session presents ideas, practicalities, strengths, weaknesses, resources and technological and operational challenges related to the implementation of validated nutrition risk screening tools in both ambulatory and acute care settings. The presentation includes operational and clinical resources for Oregon dietitians to use to encourage patient access to appropriate nutrition care to help action at their own work sites and online locations. Activities of the Oregon Academy of Nutrition and Dietetics to support these efforts will also be included.

Learning Objectives:
At the conclusion of this webinar, participants will be able to:
1. Identify resources to assist in implementing nutrition risk screening in ambulatory and acute care settings, including technology-based resources.
2. Increase understanding of challenges and opportunities when implementing operational changes related to nutrition risk screening
3. Increase understanding of resources to make available to Oregonians to better address and combat clinical malnutrition.
